Script Required for PHPbb3

I need so that a User who is logged in can access a page with a checklist in it. The list itself is contained in MySql Tables.

Below is a example of what i need

[ ] Species---------Date----------Location-------Comments ( up to 255 chars )
[ ] Kestrel
[x] Eagle-----------24/06/08-----Glasgow------Was eating a Rabbit
[x] Robin----------16/08/08------Italy ----------It was Flying
[ ] Pigeon

--- = spaces
When a user checks the checkbox, they get prompted to enter a Date, Location and comment ( optional ).

This list is for the current Year, at the start of the next year, this list is got no entries only the list of birdnames.

Also, a secondary list is avaliable to view containing a life long ticklist.

I need a PHP script to send this information to a HTML Block.

Can you do, if so how quick ?
